Practitioner Registration (with Sales Rep)

We’re glad you’ve heard about our products through one of our sales representatives.  In the interest of expediting your registration process, you can register using the last name of the sales rep to gain instant access to our products.  You’ll be able to place your first order immediately.  We’ll reach out to you to obtain a copy of your medical license after this first order, which will be required for future orders.
